  • Department or Organization (if not ECE)
  • Your student or employee number (if you have both, please provide your student number)
  • Name of your supervisor
  • Your preferred username
  • An email address that you can currently be reached at
  • Your affiliation with ECE:
    • One of the following: MASC student, PhD student, Postdoc, Faculty, Staff, Coop, Visitor, Other
